    • To support quality and effective education that starts at birth and continues throughout life;
    • To increase healthy lifestyles through health promotion and prevention;
    • To increase personal and community responsibility for a healthy community with self-esteem and a high quality of life.

  • The Foundation is committed to using solid data and measurable indicators to benchmark its efforts at improving community health.


  • The Leading Community Health Indicators Report, the first comprehensive review of health status and key issues of the tri-state, guides the selection of funding targets.


  • The most recent (2006) edition is available at the Foundation office.  It can also be viewed in its entirety at: http://www.welbornfdn.org


  • Applicants are encouraged to reference this report and supply information from additional sources as needed.

  • Health Ministry teams may include pastors, nurses, and lay members of a faith community

BASIC PARISH NURSE
 EDUCATION PROGRAM
  • 5-day curriculum endorsed by the International Parish Nurse Resource Center is the desired education for parish nurses
  • Internet-based version of this class is offered twice per year
  • Both are co-sponsored by USI Continuing Education Dept. and the Tri-State Parish Nurse organization and offer CEU’s.
  • Program cost: $550
  • Welborn Baptist Foundation offers $450 special grants to help offset the program cost.

PREVENTION PROGRAMS
  • Based upon the substantial infrastructure that has developed under the Youth First Inc. program, the foundation requires any organization (church, faith-based or secular) wishing to conduct youth substance abuse prevention activities, to make contact with Youth First, Inc. prior to developing a letter of interest.

PLEASE NOTE
  • Target 5: Early Childhood Development
  • The Foundation is not accepting requests under this target in 2008.
  • The Welborn Baptist Foundation has  defined a priority in early childhood development. At this point, this effort is Foundation-directed rather than incorporated into the Foundation’s responsive grant making program.
  • Child care providers (family home and center-based) interested in learning more about, or participating in, this initiative are encouraged to contact 4C of Southern Indiana, Inc. at (812) 423-4008 or visit the 4C website at www.child-care.org.

GENERAL OPPORTUNITY
  • Up to 20% of the funding may be directed to general projects.
  • Requests must be related to the priorities of health and education
  • Requests that strengthen organizations’ ability to sustain their mission, in relation to the Foundation’s interests, are of interest.
  • Capital requests typically fall into this category, but must be an integral part of a program that is aligned with the Foundation’s grantmaking priorities to receive consideration from the Grants Committee.

PLANNING GRANTS
  • Planning grants will be considered for activities proposed in any of the target areas. Appropriate planning activities include:
    • assessing and identifying service needs;
    • establishing community partnerships;
    • developing the program;
    • selecting services and site(s);
    • creating a start-up budget, an operational budget and a business plan
    • risk assessment

  • The Foundation is more interested in the quality of your collaborative efforts than in the quantity of organizations with which you are collaborating.

  • Successful applicants will incorporate the following characteristics into their programs:
    • Research and evidence-based practices, sometimes called “leading practices”
    • Address root causes of good health and quality of life
    • Strong partnerships
    • Community outreach and impact
    • Fit with the Foundation’s funding interests
    • Include evaluation as part of their program

RESEARCH
  • The Welborn Baptist Foundation advocates the use of research-based programs and practices in program development.


  • Simply put, a program is judged to be research-based if:
  • Evaluation shows the program produced the hoped-for positive results
  • The results can be attributed to the program itself, rather than to other factors or events
  • The evaluation is reviewed by persons knowledgable in the field.

  • Statement of Need
    • Clear description of the need, including its nature, size and scope
    • Clear identification of the root cause of the problem
    • Clear description and research base of your proposed intervention
    • Match for the Foundation’s targeted interests
  • Merit of the Project
    • Does not duplicate an existing program or service;
    • What specific actions will be undertaken to address the root causes?
    • What are your specific goals and objectives?  Can your results be measured?

    • Realistically estimate the number of persons impacted, and how their lives will be better as a result of your efforts.

TAX-EXEMPT STATUS
  • FAITH-BASED ORGANIZATIONS (Non-Church):
    • Must provide Internal Revenue Service Letter of Determination conferring 501(c)(3) tax-exempt status
      • Documents that you are exempt from paying federal income tax
      • Documents that you are not a private foundation
    • PLEASE DO NOT send your state sales tax exemption certificate

TAX-EXEMPT STATUS
  • CHURCHES
  • Affiliated churches may provide evidence of membership in a larger religious organization that has been determined to be tax exempt by the Internal Revenue Service.


  • Non-affiliated churches that do not have a 501(c)(3) letter, please contact a program officer regarding alternate means of documenting your exemption from federal taxation.

TAX-EXEMPT STATUS
  • Failure to provide documentation of tax-exempt status with a Letter of Interest will immediately disqualify the letter and cause it to be removed from consideration by the Grants Committee.

  • Future class sessions will be scheduled based upon interest/demand.

In preparing your letter….

  • Be concise.
  • Think it through. Does your idea make sense? (Is it more than just a good idea?)
  • Be realistic. Does your organization have the financial and operational ability to perform the project being proposed?
  • Are you attempting to address a root cause?
  • How broad is your project’s impact?
  • Are you seeking support for an existing program? Are you seeking replacement dollars?
  • Is this proposal a duplication of an existing service?
  • What are the potential sources of permanent funding beyond the Foundation?
